OPA Policy Violations Dashboard with User Visibility

Currently, there is no dashboard or reporting view for OPA (Open Policy Agent) policy activity. Policy definitions are visible at the account/org/project level, but there is limited visibility into policy enforcement events.
Feature Request
Provide a dashboard (or dashboard widgets) for OPA policies that shows:
- Policy violations and warnings over time
- Which user triggered the violation or warning
- Policy name and severity (violation vs warning)
- Associated pipeline, project, org, and account
- Timestamp and execution context
- Filtering by user, policy, project, and time range
Alternatively, expose this information via logs or events so it can be used in custom dashboards.
Value
This would improve governance and policy compliance by enabling teams to quickly identify enforcement trends and directly follow up with users responsible for policy violations or warnings.
