So many times, i click "Reject" thinking it's just a "Cancel" button, because it's gray and looks like a typical cancel button. the icon being an X has shared meaning with x top-right as well.

Easiet fix - make reject a different color - typically red, and different icon. Cancel button can be there also - leave it to your UX people for a comprehensive layout.
